Koi may cover almost the entire body, but should leave roughly 5% of the underlying base coat showing.

Examples
Koi may appear anywhere on the body, but must appear in at least this amount. A red or orange circle must appear on the forehead regardless of your color choices.

Koi may be layered under or over markings, but all colors must be on the same layer. Here, Sealpoint is under Koi and Sunglow is on top. Note how neither marking comes between the black, white, and red patches.

They should not have any outlines, and should consist of solid scale-like shapes. |
The easiest way to create the iridescent appearance is to create you scale shapes then airbrush an assortment of colors on them. Then set the layer to Screen or Luminosity and lower the opacity so the shapes become quite subtle.
